  In accordance with NCAA rules and regulations regarding a student-athlete’s participation in an institutional, charitable, educational or nonprofit promotion (12.5.1.1.a), the student-athlete must first receive written approval from the Director of Athletics. This statement must be signed by the Director of Athletics prior to a student-athlete’s participation.

  Student-Athlete instructions for hosting for an official visit

• Student Host Form

  Complete the following upon initiation of Official Visit and when funds are given to student host for entertainment of prospect(s)

• Camp Approval UAH Owned   or   Camp Approval Owned

  Must be completed prior to organizing or participating in any camps

• Travel Liability Release

  Must be filled to allow the student-athlete to travel outside of the team travel party
